Effective 1/1/2017 My Health Care Coordination will cease all operations, and our staff will no longer be available to assist you with your health care needs. All members eligible for Medicaid benefits will continue to receive those benefits. We have provided information below to help with your transition.
The Following statements are effective 1/1/2017:
1) Your Medicaid benefits will no longer be provided by Health Alliance, they will be provided by the State of Illinois Medicaid Program.
2) You will need to make sure that you have your Medicaid Card. You will need to have this card with you at the pharmacy and Doctor’s office.
3) If you cannot find your Medicaid card call the DHS helpline (1-800-843-6154) or visit your local DHS office.
4) The 4 prescription a month limit may apply. Your pharmacy can assist you with any prescription related questions.
